Note: This form does ask for a credit card.  That is just to save your spot for the assessment.  Nothing will be charged to your card.  If you decide to move forward with tutoring, this will be the credit card that is used.  If this makes you feel uncomfortable and you would like to chat first, call Joanne at 414-732-6838 and leave a message.  Joanne will get back to you within 24 hours.
